What is Crossways Supported Housing Ltd?

 

Crossways Supported Housing Ltd manages housing projects for single homeless people in Surrey area. Our aim is for adults who are homeless or at risk of homelessness to become independent and resilient. We want everyone to have a place that they can call home, the support they need to keep it and a country free from homelessness.  ​We work alongside both national and local voluntary organisations and statutory agencies in order to achieve our aim.

At Crossways, our supported living services might include:


  • Supported Housing Accommodation
  • Help with applying benefits
  • Support with shopping, budgeting and living skills
  • Social inclusion
  • Assistance to return to education and work
  • Help in developing and forming relationships
  • Rehabilitation
  • Facilitate discharges from hospital 


In addition of providing a safe place to live, Crossways also work closely with individuals so that they can have every chance of fulfilling their potential and can successfully move on into independent living.  We enable people to gain the skills and confidence to increase future life choices.

Our packages of care can include:


  • Daytime, evening and weekend support
  • On call assistance which available 24hours a day
  • Short breaks & respite
  • Support at times of change in a person’s life eg: moving between support services, moving out of the family home


Who can apply?

 

In order to apply to Crossways, you must be: -


  • Aged over 18; and
  • Homeless; and
  • Single (no children or partners can be housed with you).

We give first priority to those with a local connection to the Surrey areas and then accept those out of area in exceptional circumstances.

The service is available to anyone who meets the above criteria irrespective of race, ethnic or national origin, gender, sexuality or other characteristic.

 

 How can you apply?

 

You can apply for supported accommodation at Crossways as follows:-

 

  • You can self-refer
  • You can be referred by your Lead Practionner/ Care Coordinator/ Support Worker if you have one
  • By NHS inpatient hospital referrals
  • By Community Mental Health Team referrals
  • GPs Referrals
  • And other voluntary and statutory agencies referrals

What happens once my form is returned?

Once we have received your application, we will invite you for an interview and we will also assist you to view the accommodation and the facilities that it offers. We will try our best to answer any questions that you may have. After the interview, our aim is to contact you within 48 hours to inform you of the outcome of the interview – whether you are suitable for the scheme or not. When a vacancy arises, we will contact and support you to move in your new supported home.
